Important: Even if you filled out a form previously, please fill this one out again. This is specifically for next school year (2025-2026) and includes a few new questions to help us build the best program possible.

A Note on Commitment:

When you sign up for an Esports team, you're making a commitment to your teammates and to the school. Remember, your team is counting on you to show up, practice, and compete. School funds are used to pay for league fees and other expenses, so when a player doesn't participate, it impacts everyone. It wastes resources that could be used to benefit the whole team, and more importantly, it lets your teammates down. Let's make sure we're all in and ready to support each other throughout the season!
