Sports Registration Information
Register your athlete by using InTouch Registration:
1. Login to the TuHS Student web store (link is provided on the athletics website.
2. Click on register for athletics and activities.
3. Select your sports team.
4. Fill out the clearance form, pre-participation examination form and pay to play form, the transportation form online.
5. Upload Physical Examination Form.
Tigard-Tualatin School District Athletic Clearance Form: The Clearance Form is to be completed and signed by the athlete and a parent/guardian. This form gives permission for the student athlete to participate in a sport. Clearance Forms are good for one school year.
School Sports Pre-Participation Examination Form and Physical Examination Form: An athlete's physician will fill out the Physical Examination Form after parents fill out the Pre-Participation Examination form. Incoming 9th grade athletes need a sports physical dated after 5/1 of their eighth grade school year. Oregon State Board of Education requires high school athletes to have a physical completed by a doctor. Please upload the physical form when completed into the online sports registration system. A new physical is required every two years.

Transportation Form: This form gives permission for athletes to ride to athletic events with a parent if bus transportation is not available.
Pay to Play Form: At Tualatin High School, Pay-to-Play fees help cover the operating costs of our athletic programs. However, we believe sports fees should never be a barrier to participating in high school sports.We are committed to ensuring all students have the opportunity to participate in sports. Completion of this form is REQUIRED for every student athlete. Fees will be added to the athletes account after Tryouts. To qualify for reduced or waived pay-to-play and activity fees, please log into your ParentVue account to complete the Household Income Survey.
Follow these instructions to complete the form:
1. Log in to ParentVue
2. Select "Online Registration"
3. Select "Update Student Information"
4. Parents/guardians will be directed to complete the data update process and provide the income survey information.
How Will You Pay? After tryouts, you will receive a ParentSquare notification letting you know the fee has been added to your student’s TuHS Online Store account. Fees can be paid online through the store, or in person directly to our bookkeeper.
